Here's a good general discussion of the problem and solution space:
http://www.jdrosen.net/papers/draft-rosenberg-sipping-spam-01.txt

Here are some slides to review, from the first SIP.edu implementors workshop in 2004:
http://www.internet2.edu/sip.edu/200406-workshop/talks/20040616-teitelbaum-spam.pdf
http://www.internet2.edu/sip.edu/200406-workshop/talks/20040616-george-identity.ppt

The basic idea seems to come down to authenticated identity used to filter calls.

SIP identity work has continued:
Enhancements for Authenticated Identity Management in the Session Initiation Protocol (SIP)
http://www.ietf.org/internet-drafts/draft-ietf-sip-identity-06.txt
Although the draft is expired, the discussion continues at http://www1.ietf.org/mail-archive/web/sip/current/msg14936.html

Here's something a little newer (June 06)
Coexistence of P-Asserted-ID and SIP Identity
