I found the problem.  I think there might be a bug in the installer.  I
was looking in the wrong place in the IIS configuration for a bit, but I
finally found the right ISAPI configuration dialog, and the Shib ISAPI
filter specified this path:

C:\opt\shibboleth-sp\libexec

The DLL is installed at:

C:\opt\shibboleth-sp\lib\shibboleth\

(I suppose it's also possible that this path was screwed up, since this
was not a clean install)

I was also assuming you're experienced enough with all the usual stuff
to
do:

- run depends.exe against isapi_shib.dll to rule out a path issue
- make sure shibd runs (which also rules out path issues)
- use shibd -check

After that point, it's pretty much always permissions.
