I haven't run depends against isapi_shib.dll (this machine is not a
development machine, so I don't have it), but I did all of the shibd
diagnostics.  I tried a bunch of permissions variations as well, but I
haven't done so exhaustively.  I'll keep digging into that.

I was also assuming you're experienced enough with all the usual stuff
to
do:

- run depends.exe against isapi_shib.dll to rule out a path issue
- make sure shibd runs (which also rules out path issues)
- use shibd -check

After that point, it's pretty much always permissions.
