Sure, I were able to see the sub-form. Please check xml-ls.png, is that what you mean? The exist user and database were created immediately after applying.

Could you attach also service.properties file from XML-LS?
The file "ervice.properties" located in our case here:
/opt/perfsonar/services/xml-ls/WEB-INF/classes/perfsonar/conf/service.properties
is attached as ervice.properties.txt
Everything seems to be all right.
By the way, have you changed registration URL to new LS in MA?
Now the link should be
http://host:8080/xml-ls/services/LookupService
instead of
http://host:8080/XML-LS/services/LookupService

For my convenience I decided to keep the old link of the LS service in /opt/perfsonar/tomcat/conf/server.xml

- LS service is properly installed and configured
Is it? I tried to check both addresses:

the correct link is

and
http://uran.acad.bg:8080/XML-LS-1.2
for the service configuration /with default user and pass settings so far/
and both failed, but perhaps you disabled the service?
Tomcat is working because I got 404 error.
Anyway, I am sure you tested at least echo to LS with perfSONAR UI.

I've checked it and it seems to be fine
- What with RRD MA? Do you have metadata configuration file?
Yes, it is rrd-database_BREN.xml and placed in
/home/perfsonar/webapps/geant2_java-rrd-ma-x86_32-3.0rc5/WEB-INF/classes/perfsonar/conf
the RRD MA service was previously installed and tested completely with Perfsonar-UI but.
The address of this service is:
http://uran.acad.bg:8080/geant2_java-rrd-ma-x86_32-3.0rc5/services/MeasurementArchiveService
the user and pass were kept again the default ones.
