I too have been craving an "Apache" of IP telephony! The closest
thing I've found has been OSR (http://osip.atosc.org/osr.html) - a
GPL'd SIP registrar and redirect proxy server. It's based on an open
source SIP stack called oSIP, which is available at the same site.
OSR is very primitive. It would be great to see this community extend
this work (or something else) into a more full-featured open source
IP-PBX.

NIST also has some open source stuff, though I haven't looked at it
(http://www.antd.nist.gov/proj/iptel/).
