Jamey:
I am wondering if the data should be pulled from the subscriber or
location table? The subscriber will give a complete list of possible
addresses but the location table will show who is currently
registered. What do you think?

I created a simple web page for viewing subscriber names with link to
their sip uri. It works off the ser subscriber table and authenticates
against that same table. I just did basic authentication, so passwords
are clear text. I can either try to add digest authentication or move
the page to https instead of http.
It is displaying lastname, firstname for all who entered it, email
address otherwise.
