Are you running the initiator manually by redirecting to it, or just relying
on the automatic behavior? It looks like the latter, but I wanted to make
sure.

I think I failed to document it (because I didn't really think about it),
but the RequestMap properties only apply to the automatic case, not the
manual one.

Otherwise I don't know offhand, but it sounds like a bug and I'll see if I
can reproduce it.
