We've found that Broadcom work well; as Szymon notes, they're very liberal 
about which optics and DACs they accept. One downside is that the current 
Linux drivers (at least in CentOS 6, the last time I looked) don't let you 
mess with interrupt coalescing, but that's not been a big issue. Myricom 
ConnectX-3 cards also work well, but (unless something has changed with 
recent updates) Centos6 doesn't support them out of the box. Building the 
driver isn't hard, though.
