Try something like this (substitute in the sourceId and subjectId:
select *
from grouper_audit_entry_v, grouper_members gm
where string04 = gm.ID
and label_string04 = 'memberId'
and audit_category = 'membership'
and gm.SUBJECT_SOURCE = 'sourceId'
and gm.SUBJECT_ID = 'subjectId'
