I'm sorry for the delays on editing the survey.

Until last week, I didn't yet have permission to do so. It turns out
that the confluence user interface for setting permissions obscures a
very important thing: the identity of the user for whom you're setting
permissions. So George gave "Ben Chinowsky" (User Name benchinowsky)
admin permissions, but Ben was actually using a different account:
"Ben Chinowsky" (User Name
).

That is fixed now, and these users can edit now, listed by Full Name
(since we've found no way for an admin of my rank to find out the User
Name of the folks that have been given permission after it has been
granted(!)):

Ben Chinowsky, Candace Holman, Ben Chinowsky, Deke Kassabian, George
Brett, Neal McBurnett, Randy Stout

I'm eager to add others to the list - if you want edit permission,
please let me know, and include your "User Name" (ala User Name:
nealmcb, not your "Full Name") from your confluence profile.
Or let me know if you can't edit because the wrong User Name....

I also added a "Wiki sandbox" as a new child page in the PIC space,
for experimentation:

Next, I started editing the survey. I first wanted to add item
numbers, as requested, and to make it easy to import into
surveymonkey.

Therein I ran into more questions. Surveymonkey doesn't seem to have
a convenient way to import an html page as a survey, so there is a lot
of cutting, pasting and clipping out of the very question numbers that
make it easy to discuss the survey in draft form....

I also see that surveymonkey requires a "Professional status account"
($19.95/month) to include things like html links or styling in the
survey.

By the time I got to that point, Deke had cancelled the meeting.

I made an update to number the questions, to make it easier to discuss
them. I also made the easy changes, as you see if you click on "SHOW
COMMENT" just above the page content itself.

link to use cases; add google talk; add "instructional use" option;
remove "optional" from name/title;

I still haven't made some of the other changes to the sequence and
wording that I talked about last time.
