I recommend looking into the Grass Valley product line. They have solutions for almost all that you're asking about:

Input will probably be 1080i or p, 720p if possible, whatever the camera puts out, the final encoded web stream will probably not be more than 480p, with smaller frame sizes. An inexpensive but adequate solution is preferable. I looked through every page of the Markertek catalog, saw some options, did a couple of hours of Googling, but would like to hear from someone with experience with any of the above.
Also looking at the Teradek Cube, but would like some computer-based options.
