I believe it uses the local cellular network but not international and long distance. Shares the wealth.
Yes, as long as you don't use your cell phone in a foreign network. Then you'd have to pay normal (overpriced) roaming charges. Also if you call another cell phone using SkypeOut the carriers still get their revenue:

Unless it is specifically mentioned, the SkypeOut Global Rate is only for calling regular landline telephones. Calls to mobile phones are more expensive.
