Tom's last round of corrections are incorporated, the ones I agreed with at
least (most of them).

I tentatively said I "forgot" to put the Domain extension element into the
spec, but I recalled now that I debated and decided against it. I think
scoped attributes are an invention of this particular implementation, and
don't rise to the level of this document, yet anyway.

They need to be documented (along with the extension) somewhere with the
attribute material and the implementation specs, but I don't think it
belongs here, particular since the extension is only used by the acceptance
policy code, which is also not part of the spec.
