If it's trying twice, then you've got a separate problem, namely the origin
thinks you target is running 1.1 and is including a location at runtime. You
must not be passing a providerId parameter to the HS or something.

I can't explain the rest, but I wonder if Apache is blocking you. Maybe
mod_ssl is misconfigured.
