I've checked in the bulk of my non-Windows changes for this 1.0.1 point
release.

I made some internal changes so that all of the attribute configuration
could be moved into the AAP.xml file, including the web server details like
the request header to export into.

The shibboleth.ini attributes section is gone, and will be ignored if
present, and even if you use the AAP.xml file we shipped in 1.0, nothing
needs to be changed to make it work.

You do have to edit the file or use the new version I created for Apache to
work without the ShibMapAttribute commands, but I left support for that
command in. It's deprecated and will warn of this, but it does get used, so
1.0 config files should work unchanged with 1.0.1, which was a goal of mine.

The AAP interface is now aligned with the rest of the metadata, so you can
use multiple files, edit them on the fly w/o restarting, etc. Adding new
attributes to accept should be a one step process now.

I'll update the text in the deploy guide or send text to Nate depending on
how large the changes look.
