I have observed multiple cases where a package manager (one time yum, another time apt) decided I wasn't using something or found it otherwise un-needed (i.e. it was a dependency for something that got explicitly removed), and removed it.

Could you open the page in question in your browser then then send us /var/log/httpd/error_log? It looks like your host is collecting OWAMP data from browsing the various pages, looks like maybe the CGI script is hitting some type of error.
