Here's a very brief list of the ways to maintain grouper groups from external sources:
. use grouper loader to maintain memberships dynamically from external relational sources
. use grouper web services to update groups and memberships
. use grouper shell to import XML-formatted group, privilege, and membership information
. use grouper shell to run scripts of group management commands generated from external sources
. the grouper UI, for manual management, including cut and paste of lists of members, by grouper system administrators or other highly technical users

My boss and I were looking for a group-management-system to use for some institutions on campus, and we found grouper. I managed to set it up and got it running with a mysql database.
