Because the consensus during the meeting was to leave the page as it is for now, I've already committed what I had and closed the ticket before this thread started.  This should go into a new ticket.  

The fields and labels are automatically generated; I can see if it can be changed in the controller but if not, this will end up being more _javascript_.
