I've updated the ERD to represent the new tables. In doing so, I'm 
trying to remember if cmp_enrollment_attributes still needs to exist... 
I think we only need it to map LDAP and SAML attributes to data model 
attributes, but that we wouldn't actually adjust the CO enrollment flow 
to query for stuff based on the cmp attribute configuration.

Or should we not allow different CO flows to prompt for different org 
attributes? Perhaps we need a common set, at least enough to trigger 
reconciliation (which has to be platform-wide)?
