I agree with people that this is probably not a SIP problem.  You say that
you've tested H.323 and it works - but I assume that wasn't via the Vonage
gateway.  So possibilities are the Cisco box, the Vonage gateway, or the path
to Vonage.  You might want to check the path to the Vonage gateway and see if
there is loss and/or jitter.

Ben suggested placing a call to another SIP endpoint.  Unfortunately I think
that will be difficult with a Vonage box - my understanding is that they lock
you out of the box so you can't view/modify the configuration.  (Which is
reasonable if they gave it to you.)  But I think you can call it via its IP
address.  (I just tried with ours and something is RINGING - don't know which
line - maybe both?)  So, if you give Ben and I the IP address we could call it
and check the audio.  I haven't noticed any problems with our box - but I've
only used it in "the lab".  I did get a report from somebody else that audio 
was unacceptable using one with a Vonage-like service.  But it wasn't 
isolated 
to the box itself.
