I think you're confusing some SP out there by restarting, but what you
posted isn't from a single SSO/query pair.

Only way that happens is from a query coming in after a restart and the
mapping is gone.

Your IdP's handleTTL might be much shorter than it should be, but it
shouldn't be small enough to cause a problem without restarting in between
SSO and the query.
