From the beginning. The profile was mine to begin with.

By the time it was released, the draft schema was used, and it hasn't
changed since then. The namespace changed before any releases we did.

The rest, I have no idea. People can check subversion for the schema. As an
implementation, I think the initial release supported the extension but I
don't know how well.
