First, I would verify this with the Apache access log and see what it says.
The Shib aspect of this should be irrelevant, I would think. If you can
verify that there are actually three accesses logged, I would try removing
mod_shib and see what happens then. If it's not the same, I can't explain
that.
As far as explaining it, if it's really a document with no other content (no
style sheets, scripts, anything like that), I'd say you'd have to sniff the
browser's traffic to get an idea.
