The XML looks ok, but that error sounds like the old bug related to
processing subjects without a NameID. That doesn't match the XML though.

I would need to know the exact version being used to even guess, but I would
suggest pulling the latest code from svn on the 1.1 branch and testing with
that as well.

Or just trace the code and see why fromDOM isn't handling things properly.
