Things are changing in 1.4...  it is not an extension anymore, it is built 
in.  The updated document will be forthcoming.

I believe the correct way to do this is, checkout, do an "ant dist", 
customize your config files if necessary (e.g. 
conf/grouper.hibernate.properties if you don't want hsql)
Then just run bin/gsh.bat or bin/gsh.sh.
Note you need to be in the grouper_home dir, or the bin dir...
