I've just pushed a couple of commits to develop that add support for
Extended Types to Affiliation, Address, Email Address, Name, and
Telephone Number. (Previously, only Identifier supported Extended Types.)
This is a fairly intrusive change. As soon as you update past 57e1e2a930
you *must* run these steps:

(If we had more people running this in production, I'd write some form
of update script, but since we don't it's probably better for me to put
my time elsewhere.)
Given the scope of changes, I'd appreciate some additional testing, and
suggest putting this up in a dev/test environment before updating any
production installations. While I think I updated everything (except the
REST API, which I'm pretty sure is broken for these attributes), a
handful of issues usually shake out following a commit like this.
